Claiming UIF
Petro Pretorius [23 August 2008] - Claiming is actually very easy, you would need to following:
- 13-digit bar-coded ID or South African passport;
- form UI-2.8 for banking details;
- form UI-2.7; (your employer needs to complete this one)
- form UI-2.3 (application form)
- medical certificate from a doctor or birth certificate of the baby; and
- form UI-4 (follow-up form).
If you have not got a 13 digit car-coded ID, you need to apply for one as soon as possible, if you do not have a South African passport, as no claims will be paid without either of these.
Make sure that you complete all required forms before you arrive at the UIF offices this will save you time and will ensure quick and friendly assistance from the staff. Take a copy of all the below outlined forms with your documents, this ensures that all relevant correspondence is in one place and this will expedite your authorisation process. Please note that this will not replace the requirements of your employer to forward all their forms the relevant offices.

This form is completed by the employer and is a statement made in respect of the contributions you have made the UIF.

This is to be completed by your employer and a copy of this is to be provided to you for submission with the rest of the forms. This form is not available on-line and this shows an example of the form only.
